For anyone struggling with depression, anxiety, insomnia, ADHD, or other mental health conditions. Your provider takes a detailed history, provides a diagnostic assessment, and works with you to set goals and determine a treatment plan.

Psychiatric Evaluation - Initial - Virtual

The same initial evaluation, held by telehealth.

Psychiatric Treatment - Follow Up

For established patients needing follow up on medications, supplements, or other psychiatric treatment.

Psychiatric Treatment & Therapy - Follow Up

For patients already receiving both treatment and therapy from a psychiatric nurse practitioner.

Injured Worker - Initial Psychiatric Evaluation (no existing claim)

For injured workers who need an initial mental health evaluation and do not have an existing work injury claim in the State of Washington. Includes a full diagnostic evaluation and documentation for filing the claim, if appropriate.

Injured Worker - Initial Psychiatric Evaluation (existing claim)

For workers injured in the State of Washington who already have an existing claim and are referred for a mental health evaluation. Includes a full diagnostic evaluation, treatment recommendations, and related documentation.

Injured Worker - Psychiatric Follow Up

For injured workers already established with a Zoya psychiatric provider, for follow up on medications, supplements, or other treatment.
